This website is the definitive resource for everything related to Theory of Change. It is also a community forum for discussion and posting.
The Annie E. Casey Foundation
Theory of Change: A Practical Tool for Action, Results and Learning is a guide demonstrating what a theory of change looks like, what types of changes can be mapped, and how theory of change can be applied within a community setting.

Centers for Disease Control and Prevention
Framework for Program Evaluation in Public Health: This classic 1999 guide published in the CDC's Morbidity and Mortality Weekly Report (MMWR) provides detailed information about conducting program evaluations in a public health context.

The Management Assistance Program for Nonprofits
Guidelines and Framework for Designing Basic Logic Model serves as an introduction to developing logic models. At the bottom of this page are links to a variety of evaluation resources.

As you are planning or describing your program, it is sometimes useful to refer to theories that relate to health behavior change on an individual, interpersonal, or community level. Theories and Models Frequently Used in Health Promotion summarizes a variety of such theories.
Robert Wood Johnson Foundation
A Guide to Evaluation Primers orients non-experts to some handbooks and basic primers (introductory pieces) on program evaluation. These resources identify some of the central issues in evaluation and explain their importance. A Practical Guide for Engaging Stakeholders in Developing Evaluation Questions describes a five-step process for engaging stakeholders in developing evaluation questions, and includes four worksheets and a case example to further facilitate the planning and implementation of the stakeholder engagement process.
University of Kansas
Community Tool Box is a global resource for free information on essential skills for building healthy communities. The Toolkits section presents 16 links, including "Developing a Framework or Model of Change,” and "Evaluating the Initiative,” each of which includes key tasks, examples and other links to supports.
Usable Knowledge
The online Logic Model Tutorial distills everything you need to know to create a logic model into one interactive 15 minute audio-visual program.
W.K. Kellogg Foundation
The Evaluation Handbook provides a framework for thinking about evaluation as a relevant and useful program tool. It was written primarily for project directors responsible for the ongoing evaluation of W.K. Kellogg Foundation-funded projects, but it is very useful for all evaluators. The Logic Model Development Guide was developed to provide practical assistance to nonprofits engaged in initiating and completing outcome-oriented evaluation of their projects.
